This policy memorandum establishes institutional requirements for conducting and overseeing Human Subjects Research (HSR) at SLAC. Research involving human subjects must comply with Stanford University’s Human Subjects Research Program (HSRP) policies.

All human subject research (HSR) activities must be reviewed and approved by Stanford University's Institutional Review Board (IRB) prior to initiation, unless the IRB determines that the activity is exempt from IRB review or does not constitute human subject research.

In addition to Stanford University requirements, SLAC shall comply with applicable U.S. Department of Energy (DOE) requirements, including notifying the DOE Site Office and reporting to the DOE Human Subjects Research Database in accordance with current DOE directives and implementing guidance.
